TO BE USED FOR AN ADULT OR A CHILD OF THE SAME FAMILY<\/p>\r\n

THIS RESPONSIBILITY OF DISENGAGEMENT, RENONCIATION TO REMEDY, ACCEPTATION OF RISK AND INDEMNITY AGREEMENT.<\/p>\r\n

BY SIGNING THIS DOCUMENT, YOU RENOUNCE TO CERTAIN LEGAL RIGHTS, INCLUDING YOUR RIGHT TO PROSECUTE.<\/p>\r\n

PLEASE READ CAREFULLY.<\/p>\r\n \r\n \r\n

Considering the authorisation given to my child or myself to participate, totally or partially, to the aquatic activities (surf) and the non-aquatic activities at Centre Oasis Surf Inc. I follow and guaranty all the following statements:<\/p>\r\n \r\n

1. I am at legal age or the parents\/tutors having the entire legal responsibility of a minor child; and<\/p>\r\n \r\n

2. I know the nature of the activities concerned and I agree, for myself and my child, to assume the risks of serious damages, corporal injuries and in extreme case, death in participating in the aquatic activities and non-aquatic activities at Centre Oasis Surf Inc.<\/strong><\/em>; and <\/p>\r\n \r\n

3. I am personally convinced and firmly think that I am or my child in a physical, emotional and mental state to participate in the aquatic activities and non-aquatic activities at Centre Oasis Surf Inc.<\/strong><\/em>; and<\/p>\r\n \r\n

4. I understand that the consumption of alcohol and\/or drugs could harm my own or my child\u2019s capacity to participate in the aquatic activities and non-aquatic activities at Centre Oasis Surf Inc.<\/strong><\/em>. Me and my child will under no circumstance participate in any aquatic activities and nonaquatic activities under the influence of alcohol and\/or any other drugs.<\/p>\r\n \r\n

5. I perfectly understand, and I will follow and give instructions to my child, that all the rules given relating the aquatic and non-aquatic activities at Centre Oasis Surf Inc.<\/strong><\/em> must absolutely be followed and that me and my child are completely responsible for our personal security at any time.<\/p>\r\n \r\n

6. I will immediately remove myself or my child at any time during the aquatic and non-aquatic activities if; at any moment I feel the situation is dangerous for my own and my child\u2019s physical, mental or emotional state might compromise the ability in the aquatic and nonaquatic activities.<\/p>\r\n \r\n

7. If my child or myself bring out own personal equipment, it will be in perfect condition and are made to participate in the aquatic and non aquatic activities at Centre Oasis Surf Inc.<\/strong><\/em><\/p>\r\n \r\n

I UNDERSTAND AND AGREE, FOR MYSELF AND MY CHILD, THAT THE SIGNATURE OF THIS DOCUMENT WILL CONSTITUTE IN: <\/p>\r\n \r\n

1. AN ACCEPTATION AND AN ASSUMPTION OF THE RISKS associated in the participation of aquatic and non-activities at Centre Oasis Surf Inc., even if in the risks come from negligence, an aggravation of injuries caused by the operations and procedures of Centre Oasis Surf Inc.<\/strong><\/em><\/p> \r\n \r\n

2. A COMPLETE, DEFINITIVE AND FINALE CLEARANCE OF ANY RESPONSIBILITY AND A RENUNCIATION IN ANY RIGHT OF ACTION that i could have in the future against Oasis Surf inc.<\/strong><\/em> or any director, employee, guide, representative, publicist, sponsor, volunteer and the other participant for any loss, damages, injuries or expenses that me or my child could have during the participation of the aquatic and non aquatic activities INCLUDING IF THEY ARE CAUSED BY NEGLIGENCE, BREACH OF CONTRACT OR BREACH OF ANY LEGAL DISPOSITION; and<\/p>\r\n \r\n

3. A RENUNCIATION AND A CONSENTIENT to renounce to the right of prosecuting Centre Oasis Surf inc.<\/strong><\/em> towards any loss, injuries, expanses or damage of any kind even if it is coming or resulting from my own or my child\u2019s participation in the aquatic and non-aquatic activities; and<\/p>\r\n \r\n

4. AN ENGAGEMENT TO NOT PURSUE any legal charges, court fees, responsibilities, damages or cost of any nature that could be caused by any actions or negligences of Centre Oasis Surf inc.<\/strong><\/em> and also any person associated with the aquatic and non-aquatic activities; and<\/p>\r\n \r\n

5. ACCEPTING that the present engagement must be understood following the laws of the province of Quebec, where the aquatic and non-aquatic activities take place; and<\/p>\r\n \r\n

6. An agreement to the information given in this engagement with the insurance coverage and the confidentiality policy of Centre Oasis Surf inc<\/em><\/strong>. <\/p>\r\n \r\n
